Responsibilities

Registered Nurse- On-Call Status (PRN)

Location: Home Care Department

Position Status: On-call/ PRN

Schedule: Scheduled as needed; includes a weekend/holiday on-call rotation

Pay Range: $31.78 - 44.50 (DOE) - non-benefitted

 

The Registered Nurse will work as a member of our RiverView Home Care team providing care in both the home and Customized Living setting. The RN Clinical/Case Manager will utilize the nursing process to provide skilled care to clients in their homes and is responsible for completing and documenting client visits as per RiverView Home Care, CMS requirements, and The Joint Commission standards. Serves individuals from infancy to the elderly adult population to meet physical, mental, and emotional needs of our clients.

Primary responsibilities

  • Performs client in-home visits for; Nurse Visits, Recertification visits, Resumption of Care visits, Admission visits, Discharge visits, other Follow-up visits, and Supervision visits.
  • Establishes client care plans for each patient's medical, nursing, and rehabilitative needs.
  • Documentation per current standards and policies
  • Perform data collection and obtain accurate information
  • Effectively delegate tasks and supervise LPN and CNA as needed
  • Collaborates with other healthcare professionals to coordinate patient care and ensure optimal outcomes
  • Adhere to ethical and legal standards
  • Ensure continued competence and updated field knowledge

Qualifications

Required:

  • Current State of Minnesota license as a Registered Nurse/RN
  • Current Drivers License and dependable transportation is required. 
  • Current American Heart BLS certification. If not held currently, training will be provided and certification must be obtained within 4 months from the date of hire.

Preferred:

  • Two years+ experience in a clinical or home health setting.
  • Experience with EPIC EMR.
