RiverView Health operates a 25 bed Critical Access Hospital, RiverView Recovery Center; a chemical dependency outpatient treatment program, RiverView Home Care and five primary care and specialty clinics in the hospitals service area.

Responsibilities

Physical Therapist (PT)- On-Call Status (PRN)

Location: Home Care Department

Position Status: PRN

Schedule: Scheduled as needed.

Pay Range: $44.40 - $62.17 /hr (based on experience)

 

The Physical Therapist will work as a member of our RiverView Home Care team to evaluate and treat patients in their home. The Physical Therapist is expected to effectively and professionally interact with patients, family members, and other health care team members while maintaining RiverView Health standards of behavior. The Physical Therapist will establish patient-driven and specific goals for improved outcomes for the individual. Home Care hours are flexible and accommodating and does not require weekend hours.

Primary responsibilities

  • Complete client visits and assessments/re-assessments as per physician order and Home Care Policy
  • Participate/develop and implement care plans and participate in care conferences as requested
  • Identify patient’s needs (physical, cultural, and psychosocial) and exhibit compassion, caring, and empathy to individual differences
  • Provides guidance and education to patients and their families on adaptive techniques and equipment
  • Document and complete billing according to current standards and policies
  • Perform data collection and obtain accurate information
  • Effectively delegate tasks and supervise physical therapist assistants as needed
  • Collaborates with other healthcare professionals to coordinate patient care and ensure optimal outcomes
  • Adhere to ethical and legal standards
  • Ensure continued competence and updated field knowledge

Qualifications

Required:

  • Current State of Minnesota license as a Physical Therapist/ PT.
  • Current Drivers License and dependable transportation is required. 
  • Current American Heart BLS certification. If not held currently, training will be provided and certification must be obtained within 4 months from the date of hire.

Preferred:

  • Two years+ experience in a clinical or home health setting.
  • Experience with EPIC EMR.
  • Advanced certifications in Geriatrics (GCS) or Orthopedics (OCS).
