Responsibilities

Acute Care Coordinator & Case Management

 

Location: Crookston Hospital
Schedule: Full-time (.9 fte)
Scheduled hours: 72 hours per pay-period

Pay Range: $37.98 - $53.18 /(hourly) DOE. Non-exempt, subject to OT

 

Job Summary: The Acute Care/Case Management Coordinator will play an active role in the Med-Surg, Critical Care, and Emergency Department (ED). The Acute Coordinator will oversee skills and orientation nursing staff, assist with facilitation of staff meetings, and will be responsible for auditing of charts & assisting with performance improvement.  The coordinator will prep, plan, manage and execute the staff schedule regularly.  The coordinator will assist with patient care as needed and will work in the lead role/case management role primarily with some dedicated administrative time for coordinator duties.
 
The Acute Care Coordinator may be delegated supervision responsibility of team members, including but not limited to assisting with performance reviews, managing vacation and time-off requests, etc.  The coordinator will be a part of the peer interview process for final approval of all new hires.  
 
The coordinator’s task/roles are not limited to the above and they will be subject to complete all other duties as assigned.
 
In the case management role the employee will be responsible for all case management/utilization review needs as they arise. These needs could be but are not limited to: swing-bed review and acceptance, utilization review, discharge planning, consulting in ED or other areas of the organization as consulted, daily census, vulnerable adult reporting etc. The case management/UR role will have 2/6 weekends on call (remote work only, estimated time < 1 hour/day). 
 
This position will currently be supported by a 0.6 (3 days per week) case manager. 

Qualifications

Minimum(s):

  • Current State of Minnesota RN license.
  • Minimum of 3 years acute care experience.
  • Current American Heart BLS certification is required. If not currently certified, training will be provided, and certification must be obtained within 4 months of date of hire.
  • Current American Heart ACLS certification is required. If not ACLS certified, we will train within the first year.
  • Current American Heart PALS certification is required.  If not PALS certified, we will train within the first year.

 

Preferred(s):

  • Bachelors of Science (BSN) Degree in Nursing
  • Critical Care, Emergency, Medical, Surgical or Labor & Delivery experience
  • 2+ years Case Management Experience or ACM (Accredited Case Manager) or CCM (Certified Case Manager) certification
