    RiverView Health operates a 25 bed Critical Access Hospital, a 24 bed hospital attached memory care skilled nursing facility, RiverView Recovery Center; a chemical dependency outpatient treatment program, RiverView Home Care and five primary care and specialty clinics in the hospitals service area.

    RiverView Care Center has an opening for a full-time (80 hours every 2 weeks) Activity Director/Social Services Designee.


    The Activity Director/Social Services designee is responsible for the overall operations of the Care Center Activities Program and serves as the Social Services Designee.  The Director leads quality programs to enhance the physical and mental well-being of the Care Center residents and coordinates resident admissions to the facility.  This position reports to the Care Center Administrator. 


    Duties include:

    • Provide supervision and guidance to Care Center Activity Aides.
    • Conducts individualized assessments of each resident's activity and psychosocial needs.
    • Develops an individualized care plan for each resident, to include development of individualized goals for quality participation.
    • Hire, coach, train and evaluate qualified team members to deliver Activities programs.
    • Schedule and participate in resident Care Conferences and provide timely documentation in the resident record.
    • Ability to act as first point-of-contact for admission referrals.
    • Be a resident advocate and provide education to team members regarding the Resident Bill of Rights and the Vulnerable Adult Act.

    • NCCAP - Activity Director Certified/Provisional; or State of Minnesota licensed OT or COTA; or within the last 5 years and has 2 years experience in a social recreational program with 1 year full-time in a patient activities program, in a health care setting is required. 
    • Current American Heart CPR and Basic First Aid certifications is required.
