RiverView Health, is a community owned, membership based non-profit organization that was formed in 1898 and continues to be the sole community hospital in Crookston, MN.

 

RiverView Health operates a 25 bed Critical Access Hospital, a 24 bed hospital attached memory care skilled nursing facility, RiverView Recovery Center; a chemical dependency outpatient treatment program, RiverView Home Care and five primary care and specialty clinics in the hospitals service area.

Responsibilities

Riverview Health has an opening for an on-call. The Physical Therapist is clinically competent, licensed, professional of his/her discipline who assumes responsibility and accountability for a group of residents/patients through the process of evaluation, ongoing assessment, planning, and implementation of therapy interventions. Effectively and professionally interacts with residents/patients, family members, responsible party and other health care team members, while maintaining RiverView Health standards of behavior guidelines. Complies with regulatory standards of the American Association of Physical Therapy, the Department of Health, and any other professional guidelines or standards. Settings may include inpatient care, skilled nursing facility, sub-acute care, outpatient care, home care, and other contracted facilities. Completes evaluations/treatments with people of all ages from pediatrics to geriatric patients.

 

  • Perform initial assessment on residents/patients and re-assessments as per medical provider order and within department policy.
  • Develops and revises plan of care as indicated by resident/patient diagnosis, physical and cognitive status, functional goals/outcomes, and discharge plan.          
  • Provides treatments to meet individual’s needs (physical, cultural and psychosocial) per plan of care.  
  • Patient/Resident/Family education: Plans for and provides resident, family and caregiver education, identifies learning needs, clearly communicates information, and documents response to education provided.        
  • Document and complete billing according to current standards and policies.    
  • Discharge Planning: Plans for resident/patient discharge, anticipates long term and short term resident/patient needs, collaborate with interdisciplinary team, and assesses resident/patient prior to discharge as needed.          
  • Delegate and collaborate in providing resident/patient care according to licensure and skill levels, follows up to ensure success.
  • Professional Growth: Attends meetings, workshops, seminars and participates in committees as requested/required by facility.              
  • Completes all other duties within scope of practice.    
  • Assists in maintenance of equipment, tools, supplies and inventory.        
  • Provide educational in-services and participate in marketing activities as requested.

Qualifications

  • Current Minnesota state licensure in Physical Therapy, which must be in good standing.
  • Master’s or Doctorate Degree in Physical Therapy is required.  
  • Current American Heart CPR certification.
